- Photo identification and your dental insurance card, if you have coverage.
- A current medication list and information about allergies or relevant medical conditions.
- Your main concerns, questions, and any treatment recommendations you would like to discuss.
- Information about recent dental X-rays or records. Call us before your visit so we can arrange an appropriate way to obtain them.
Paperwork and Records
When scheduling, ask our team how to complete your intake information and share records securely. Please do not send medical histories, insurance numbers, X-rays, or other sensitive information through ordinary email or the general website contact form.
What Happens at the Appointment?
We begin with your concerns and relevant medical and dental history. Dr. Dawson evaluates your teeth, gums, and bite and recommends any imaging or additional evaluation appropriate to your needs. The visit is tailored to the reason you are coming in; cleaning and treatment timing depend on the findings and the appointment scheduled.
